
Inclusion intervention tracker to improve communication home and help with tracking impact of home-school relationships with participation at interventions.
Hello and thank you for taking the time to look at my resource. Just a little bit more information: This is a tracker, with accompanying letters, which improve the communication of targeted interventions. At each stage, there is a letter to help communicate the impact of the intervention and whether there need to be more sessions or to request support from home.
Green = everything is good and improvement has taken place. There is no need for any more intervention on that area.
Amber = there need to be a few more sessions to help with improvement.
Red = lack of attendance/engagement means the student is not showing progress and school are requesting help from home at encouraging the young person to attend.
Blue = exploring a potential SEN. This one is for a young person who is not already identified as having a SEN.
